A Home Infusion Staff Pharmacist position is available in Pittsford, NY, offering a direct hire opportunity for experienced pharmacists skilled in clinical and infusion pharmacy services. Schedule: Monday - Thursday, 8:00 AM - 5:00 PM

Responsibilities include:

• Reviewing infusion orders and patient therapies to ensure accuracy and safety
• Participating in clinical rounds to support comprehensive patient care
• Performing sterile compounding and total parenteral nutrition (TPN) review
• Maintaining detailed patient records and individualized care plans
• Ensuring compliance with USP standards and cleanroom procedures
• Training and supporting pharmacy staff to uphold quality standards
• Driving operational improvements for enhanced efficiency and patient outcomes

Qualifications:

• Current licensure as a Registered Pharmacist in New York State
• Minimum two years of recent acute care pharmacy experience OR at least one year in home infusion therapy preferred
• Strong knowledge of intravenous admixture services and intravenous therapy
• Ability to assess and monitor drug therapy in collaboration with physicians, nurses, and other healthcare professionals to meet the clinical needs of home care patients
