Social Services Designee

Blue Springs Wellness & Rehabilitation is seeking a compassionate and driven Social Services Designee to support the emotional, social, and psychological well-being of our residents and their families. Key Responsibilities Conduct psychosocial assessments and contribute to individualized care plans Support residents and families through transitions into long-term care and memory care Complete MDS-related documentation accurately and in compliance with regulations Advocate for resident rights, dignity, and self-determination Coordinate discharge planning and connect residents with community resources Collaborate with nursing, administration, and the interdisciplinary team Assist with survey readiness and regulatory compliance efforts Qualifications Certified, licensed, or registered in the state of practice (required) Bachelor’s degree in Social Work or a related Human Services field Minimum of 1 year of social work experience in a healthcare or geriatric setting Skilled Nursing Facility (SNF) experience preferred Nursing experience in a SNF is a plus Why Work at Blue Springs?
