Why this role is differentYou will carry a dedicated panel of roughly 150 to 180 older adults across a small group of assisted living and memory care communities on Cape Cod, and you will see them where they live. No clinic, no waiting room, no fifteen-minute slots. Your patients know your name and you have the time to practice the kind of geriatric medicine most nurse practitioners went to school for. Hughes Health & Wellness is a six-year-old, nurse-practitioner-owned practice serving sixteen communities on the South Shore and Cape Cod, with a clinical focus on dementia care. We are the only provider in several of our buildings, we admit new patients within one day, and we are growing. The role Provide primary care house calls to a dedicated panel in assisted living and memory care communities, typically 10 to 12 visits per day, Monday through Friday, with a schedule you help design Assess and manage dementia symptoms with a person-centered, non-pharmacological-first approach, and educate community staff on behavioral triggers Manage chronic conditions, medication reviews, acute concerns, and goals-of-care and advance care planning conversations with patients, families, and health care proxies Coordinate care with community staff, visiting nurse agencies, hospice, and specialists so patients are treated at home whenever it is safe to do so Complete same-day documentation in Tebra (our EHR) with Chartnote AI scribe support, following the practice's written coding and documentation standards How we support you Clinical mentorship. A Lead Nurse Practitioner (twelve years running a primary care practice) who mentors every clinician: ride-along days, monthly chart review with written feedback, and a first-60-day plan Real administrative support. A dedicated admissions coordinator handles intake, consents, and scheduling, and a clinical support nurse handles records requests, lab and imaging orders, referrals, and licensing paperwork, so your day is patient care Structured onboarding. You begin with paid orientation while your Medicare and insurance credentialing is completed, including building introductions with our community relationship liaison Geography that makes sense. Your buildings are clustered on Cape Cod so your driving is short and your days are predictable Compensation and benefits Orientation pay. $65 per hour during a paid 60-day orientation and credentialing period Production pay after orientation. $50 per completed patient visit, with a guaranteed minimum of $2,000 per week. At 10 to 12 visits per day, expected earnings are $125,000 to $155,000 per year Mileage. Reimbursed at the IRS standard rate for travel between communities Professional liability insurance. W-2 employment with payroll tax withholding; no quarterly estimated taxes Qualifications Master's or doctoral degree in nursing; board-certified FNP or AGNP (AGPCNP) Active, unrestricted Massachusetts nurse practitioner license and DEA registration (or a firm date for both; we cannot begin credentialing without them) Preferred: two or more years in geriatric primary care, assisted living, skilled nursing, home health, or hospice, and comfort managing behavioral symptoms of dementia Comfortable practicing independently in the field with remote collaboration, and able to see 10 to 12 patients a day once oriented Valid driver's license, reliable vehicle, and willingness to be based on or near Cape Cod A genuine commitment to older adults and to dignity in aging Hughes Health & Wellness is an equal opportunity employer.
