Description

• Conduct evaluations for patients across both inpatient and outpatient settings while crafting tailored treatment plans.
• Implement physical therapy interventions aimed at improving mobility, strength, balance, and overall functionality.
• Work closely with nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes.
• Educate patients and their families on therapeutic objectives, discharge planning, and home exercise routines.
• Maintain detailed documentation of evaluations, treatments, and patient progression in line with facility protocols and regulations.
• Engage in care team rounds and interdisciplinary discussions to ensure cohesive patient care.

Education

• Doctorate in Physical Therapy (DPT) or a related degree from a recognized program.

Certifications

• State licensure as a Physical Therapist.
• CPR certification is an asset but not mandatory.
