Physical Therapist - Per Diem

Role Overview: The PRN Physical Therapist provides home health services by assessing patient needs, developing treatment plans, and implementing interventions to improve mobility, reduce pain, and maintain functional capacity. Key Responsibilities Evaluate patients' functional status and assist in developing individualized therapy plans in collaboration with physicians. Deliver therapy services within the scope of practice, adhering to state laws and care plans. Monitor and report patient responses and condition changes to the healthcare team. Perform treatments aimed at pain relief, functional restoration, and maintaining maximum independence. Educate patients, families, and team members about care plans, progress, and functional limitations. Complete evaluations, progress notes, and develop plans of care according to regulatory guidelines. Report outcomes, goals, and projected visit frequency. Operate effectively in diverse home environments, ensuring equitable care regardless of patient background. Qualifications & Skills Current Physical Therapy license in the practicing state. Valid driver's license, reliable transportation, or access to public transit. Preferred CPR certification or ability to obtain within 90 days.
