- # Transitional Coordinator (IITS) - CAApplylocations: Lakewood - 3950 Paramount Blvd - Suite 107time type: Full timeposted on: Posted Yesterdayjob requisition id: R025414If you are a current YAI employee, please click this link to apply through your Workday account.**Key/Essential Functions & Responsibilities*** Collaborate with DDS and the Regional Centers on the Intake and Referral Process.* Facilitate and lead Intake/admission meeting.* Collaborate with the Clinical Coordinator, Consultants, and other members of the IDT in the development and implementation of the IPP and Person Centered Plan and transition plans* Assist in conducting Risk Assessments in collaboration with the Director and Clinical Director* Coordinate with Regional Center Liaison and Service Coordinator on accessing services in the community as per IPP and Transition Plan* Support the person pre transition, during transition and post transition with at a minimum of monthly visits or more frequently as necessary.* Performs all other duties, as assigned.**Pay Rate:** $87,560.30 annually**Minimum Qualification Requirements** ***including education, experience, licensure/certification, etc. and essential physical capabilities (e.g. lifting, assisting lifting, standing, etc.)**** A Master’s Degree in Psychology, Social Work, Counseling and five years of experience in the past 7 years working with people with IDD with behavioral/mental health concerns with criminal and/or forensic involvement.* Extensive experience in the development and implementation of Individual Program Plans and person-centered plans for people with IDD with criminal and forensic involvement.* Extensive experience in linking people with IDD with complex behavioral/mental health and forensic needs to community supports and resources.**Preferred Qualification Requirements** ***(desired requirements beyond MQRs above)**** Credentialed Alcoholism and Substance Abuse Counselor (CASAC) credential
