Nurse Practitioner - Post-Discharge Care Coordination

Wyckoff Heights Medical Center in Brooklyn, NY is seeking a Nurse Practitioner to serve as Clinical Coordinator in the Post-Discharge Follow Up program under the 1115 Medicaid Waiver. The role involves coordinating bedside touchpoints, directing the Post-Discharge and Scheduling Coordinators, and conducting a mix of virtual and in-person visits.
