Care Manager LCSW (Everett, WA) The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager provides professional, comprehensive patient‑centric care management services for at‑risk patients in an acute care environment. Responsibilities include assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.
