Acute Hospital Case Manager – Acute Care

Schedule: Weekend requirement: At least 2 weekend shifts in a 4-week schedule. Holiday requirement: Must commit to working 3 of the 5 holidays.

Requirements: California state license required (pending licenses accepted). 2 years of experience in acute care hospital inpatient case management. Knowledge of InterQual and Epic required. Experience in care coordination, admission reviews, and discharge planning.

Roles & Responsibilities: Coordinate patient care and facilitate discharge planning in an acute care setting. Conduct admission criteria reviews and ensure compliance with insurance authorizations. Assess patient needs and work collaboratively with interdisciplinary teams. Manage a daily caseload of 20-22 patients, ensuring effective communication with patients and families.

Compliance / Notes: Pre-approved time off: One per contract. Dress code: Business casual with lab coat, or dark solid scrubs with lab coat. LA fire card required (within the last 30 days).
