Applies the nursing process to assess pertinent data utilizingpatient, family/designated caregivers and interdisciplinary input. In collaboration with the patient, family, designated caregiversand interdisciplinary team the nurse develops and implements a planof care with identified outcomes for the patient. In collaboration with the patient, family/designated caregivers andthe interdisciplinary team patient progress is evaluated relativeto the plan of care. The plan is modified and adjusted to meetidentified outcomes and patient preferences. Completed timely and comprehensive patient documentation, inaccordance with service area standards. Applies Evidence Based Practice and Quality of Care Standards tonursing practice. Applies principles of Safe Patient Care Initiatives to delivery ofpatient care Demonstrates stewardship of organizational resources and applies tonursing practice. Connects the practice of nursing to service area/organizationinitiatives through committee/project participation and attendanceof service area/organization meetings. As a member of the healthcare team looks for mechanisms to improve efficiency and patientcare delivery. Demonstrates a commitment to professional development throughcompletion of mandatory education and annual service areacompetencies. Demonstrates punctuality and consistent attendance record. Appropriately delegates and provides supervisory oversight forsupport staff roles. Demonstrates professionalism with a consistently supportive,positive demeanor in workplace interactions.
