Care Coordinator For Cancer Prevention

The Care Coordinator for Cancer Prevention (temporary) primary responsibility is to increase preventative health screening rates and preventive health services within Sea Mar medical clinics. The Care Coordinator will act as a support to clinicians to navigate patients through each step in the process of any abnormal breast cervical and colorectal cancer screening results, assisting patients in accessing recommended follow-up care, including additional imaging, tests and appointments at tertiary care centers. The Care Coordinator will use supportive communication to motivate and navigate patients to overcome obstacles in completing follow-up testing and accessing specialty care. This position will maintain on-going communication with patients, clinical staff (Primary Care), and community partners to assure the patients' timely and appropriate care.

  • Review electronic health record charts and patient lists to identify patients in need of important health screenings and services.
  • Audit chart records in Epic & Care Everywhere for cancer screening and follow up reports
  • Navigate patients with referrals related to breast, cervical and colorectal abnormalities.
  • Schedule appointments/ referrals with appropriate provider(s) for cervical, breast and colon cancer screening diagnosis, and treatment.
  • Request medical records from outside care providers and upload to ensure accurate and complete medical record.
  • Navigate patients with outside referrals to specialty care.
  • Contact patients over the phone who are due for cancer screening tests and prepare mail outs with appropriate materials and patient identifier information.
  • Consider and strategically address the "social determinants of health" which may be delay or otherwise interfere with timely and appropriate follow-up services. This includes contacting and reminding patients to schedule appointments, anticipatory guidance about expected tests and procedures, and navigate on how to arrive at scheduled appointments on time and prepared.
  • Collaborate with Sea Mar medical clinical staff, research teams, and other stakeholders in the planning and implementation of educational, training and quality improvement efforts for reproductive cancers.
  • Work with Sea Mar staff to rollout additional preventive health screening and recall efforts to patients.
  • Attend trainings and learning opportunities that expand content knowledge and best practices.
  • Support quality improvement efforts regarding cancer screening, tracking and follow-up.
  • Track appropriate patient-level quality outcomes for monitoring and evaluation efforts.
  • Assist with Quality Improvement projects

Qualifications

State the minimum qualifications required to successfully perform the job. These qualifications are necessary for someone to be considered for the position.

Required Qualifications include:

  • Perform other related duties as assigned.
  • Must be able to work both independently with minimal supervision and as a collaborative team in a dynamic work environment.
  • Must be able to use good judgment, meet deadlines and prioritize workloads. Must be able to maintain professionalism and appropriate boundaries in all interactions.
  • Must be detail and process-oriented with experience using electronic recording data system (i.e. Epic) and various Microsoft Office applications such as Word, Excel, Access, Power Point, Outlook and Internet applications.
  • Ability to think strategically to work though barriers with patients around medical access and procedures.
  • Strong communication skills and ability to connect with patients by phone.
  • Strong organizational and time management skills.

Preferred Qualifications include:

  • Working knowledge of preventative cancer screening and its importance.
  • Knowledge of patient care processes, clinical procedures, and medical terminology, particularly regarding breast, cervical and colorectal health.
  • Ability to work with electronic health records, create messages, look at labs, review patient encounters and, as appropriate, update the medical record with historical data.

Education, Certificates, Licenses, Registrations, and Medical Screening

  • High school diploma or equivalent required.
  • Bilingual in English/Spanish Required.
  • Minimum of one year of experience in nursing, medical assistant or related clinical care delivery, or community health outreach and education.
  • Pre-hire and annual employee health screening required.
  • Initial TB test at hire and annual retest.
  • This person must receive a satisfactory background check from the Washington State Patrol and maintain a valid Washington Driver's license and automobile liability insurance coverage.
  • Annual influenza vaccine required. Only exception is for employees with a medical or religious exemption approved by Administration. Employees with an approved medical or religious exemption must wear a mask at all times during the flu season.

Work Environment

  • While performing the duties of this job, this employee is regularly required to communicate by mail or by telephone with patients.
  • While performing the duties of this job, employee is regularly required to sit and work at a computer for long periods, and work in a shared space with ambient noise and other distractions.

Hourly - Hourly Plan, 21.88 USD Hourly
