Requirements

• Current state license as a Registered Nurse - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• CHPN certification - Preferred
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
• Bachelors of Science in Nursing - Preferred

Responsibilities

• Administer skilled nursing care for clients of all ages in their place of residence.
• Assess patients in stable and emergent situations to identify their needs.
• Develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and their families regarding care, diet, medications, and home safety.
• Supervise and provide clinical directions to home health aides to ensure quality service.
• Provide effective communication and serve as a liaison among patients, families, and healthcare team members.
