Sign‑on Bonus $5,000 sign‑on bonus payable in two installments: 50% after 90 days and 50% after one year. Employment Details Department: Unknown Employment Status: Full time Hours per week: 40 Schedule: Monday through Friday – new clinic in Zion IL Pay Range: $41.10 – $61.65 per hour Eligible for Benefits: Yes Education and Experience Required Doctorate in Physical Therapy. Previous PT experience required; Pelvic Health a plus. Recognizes needs and behaviors of specific age groups of patients treated. Comfortable working at a site with no gym, collocated with physicians. Major Responsibilities Perform evaluations, gather pertinent patient historical information, administer diagnostic tests, and analyze results to provide accurate PT diagnoses. Implement PT programs: develop functional long‑term goals, set and revise measurable short‑term goals, provide consistent treatment, use therapeutic exercises and modalities, fabricate/adapt orthotics, and establish discharge plans. Communicate patient‑related information to nursing and other healthcare providers; prepare and maintain patient reports and progress notes. Provide patient and family education, including assessment of knowledge, delivery of teaching, home exercise instructions, and evaluation of teaching effectiveness. Understand managed care impact on PT services by staying abreast of coverage and benefits and adjusting treatment plans accordingly. Participate in professional development, including in‑services, conferences, and sharing continuing education with peers. Engage in departmental administrative processes, process improvement, quality assurance, and safety programs. Assume other related responsibilities as assigned. Licensure Licensed Physical Therapist in Illinois Knowledge, Skills & Abilities Required Good communication skills. Competent in application of a variety of modalities and therapeutic techniques. Eligible for licensure to practice in Illinois. 0–3 year credential for pediatrics. Physical Requirements and Working Conditions Exposure to hazardous materials and life‑threatening diseases. Direct contact with body fluids. Lifting and transferring requirements: lift 50–100 lbs occasionally; 25–50 lbs frequently; 10–20 lbs constantly. May lift up to 35 lbs without assistance; larger lifts require patient handling equipment and a second associate. Assist, escort, transfer, and respond to patient needs for extended periods. Weekend, holiday, and early evening services. Off‑campus location.
