This role offers the opportunity to build meaningful one-on-one relationships with patients while delivering high-quality clinical care in a home-based setting. The Registered Nurse will be responsible for assessing patient needs, developing and implementing care plans, coordinating services with interdisciplinary team members, and ensuring compliance with regulatory and physician-directed care requirements. Key Responsibilities Conduct patient assessments and develop individualized care plans. Deliver skilled nursing care in accordance with physician orders, agency policies, and professional standards. Monitor patient progress and adjust care plans as needed to support optimal outcomes. Complete medication reconciliation and collaborate with physicians regarding treatment plans. Ensure accurate, timely, and compliant clinical documentation. Educate patients and caregivers using appropriate teaching methods and document educational interventions. Coordinate care with physicians, case managers, therapists, home health aides, and other healthcare professionals. Monitor authorization requirements and maintain compliance with payer guidelines. Report patient concerns, changes in condition, and care issues promptly to appropriate personnel. Supervise home health aides and complete required supervisory visits within regulatory guidelines. Participate in patient care transitions and communicate care plans effectively among team members. Maintain productivity expectations and complete assignments independently and accurately. Stay current with nursing standards, clinical guidelines, and best practices. Perform additional duties as assigned. Required Qualifications Associate Degree in Nursing (ADN) or Diploma in Nursing. Minimum of six (6) months of Registered Nurse experience OR at least one (1) year of Licensed Practical Nurse experience. Experience in home health or hospice care preferred. Basic computer proficiency and documentation skills. Active New Jersey Registered Nurse (RN) license. Current AHA Basic Life Support (BLS) for Healthcare Providers certification. Preferred Qualifications Bachelor of Science in Nursing (BSN). Prior home health nursing experience. Strong care coordination and patient education experience.
