Our Mission: to advance and redefine healthcare.Full Time Benefits:7 HolidaysGenerous PTOMedical, Dental and VisionCompany Paid Life & AD&D InsuranceVoluntary Life & AD&D InsuranceVoluntary STD & LTD Insurance401(k) immediate enrollment with matchTuition ReimbursementReferral Bonus ProgramRequired Qualifications:License to practice Physical Therapy within the stateSuccessful completion of the National Certification Examination for Physical TherapistExperience with long-term care and/or geriatric care is preferredMust pass a criminal background checkPossess strong communication and interpersonal skillsMust have the patience, empathy, and compassion needed to care for elderly patientsAbility to work cooperatively with other healthcare professionalsRequires proficiency in technology-driven patient care and HRIS and timekeeping/attendance software.Proficient in reading, writing, and speaking English.Major Duties and Responsibilities (include but are not limited to):Optimize the physical abilities and skills of patients by performing or supervising therapy staff in the following activities: initiating referrals; evaluating and treating appropriate patients; complying with Minimum Data Set procedures; effectively participating in the care planning process; appropriately documenting progress toward identified objectives and communicating patient status and needs to the patient, the patient's family, the staff and other professionals as appropriate.Conducts screenings of patients at regular intervals to determine need for intervention/treatment.Establishes individual patient schedules in conjunction with other team members.Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.Coordinates treatment plan with patient, family and other team members.Implements and conducts treatment as outlined in the treatment plan.Monitors patient's response to intervention and modifies treatment as indicated to attain goals in accordance with physicians' orders.Develops appropriate home or community programming to maintain and enhance the performance of the patient in his/her own environment.Documents result of patient's assessment, treatment, follow-up and termination of services.Adapts treatment to meet the individual needs of the patientsProvides/recommends/fabricates adaptive devices, orthotics or prosthetics.
