Position Profile The Admission Coordinator develops and maintains relationships with referral sources and provides patient assessments in various settings to facilitate a smooth transition into Hospice of the Valley programs. Care delivery primarily occurs in hospital settings and skilled nursing facilities. Responsibilities § Facilitates smooth transition and admission of patients from health care settings into HOV programs. § Promotes HOV services and programs. § Develops and maintains therapeutic relationships. § Plans care and intervenes effectively. § Provides effective patient/family/caregiver teaching. § Supports the death/dying process. § Collaborates in providing patient care. § Creates timely and accurate documentation. § Utilizes resources effectively and efficiently. § Adheres to scope of practice/HOV standards and facilitates continuously improved processes/services. § Maintains and enhances professional skills. § Adheres to high standards of personal and professional conduct. Minimum Qualifications § Graduate of accredited school of nursing. § Minimum one year of recent experience in medical, surgical or critical care as a professional nurse. § Strong clinical assessment skills. § Excellent communication and interpersonal skills. § Solution driven, creative and resourceful problem solving skills. § Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through.
