This position will be located at 2 Wildwood St. New Britain, CT 06051. Sunday - Thursday 3:30pm-11:30pm. **Compensation Range:** The salary range for this position starts at $20 an hour. *Compensation for each position is determined not only by years of relevant experience, education, and skills, but also by maintaining internal pay equity. CMHA also takes into consideration our benefits and paid time off package to provide a well-rounded and competitive approach to the applicant’s overall compensation.***Position Summary**: The Case Manager support, teach, advocate, and empower clients with mental health, medical and Substance Use needs. This includes developing person-centered recovery goals to live, learn, socialize, work and maintain health and wellness in the community.**Essential Responsibilities:*** Provides advocacy, support and guidance with contacting external organizations (Social Security, DSS, DCF, DMHAS, etc).* Educates clients to utilize emergency and community resources to help meet basic needs.* Works with clients to teach life skills necessary to live independently in the community. This includes assisting with finances and entitlements, supporting the development of activities of daily living, nutrition, housing maintenance and repair, and prompting to encourage task completion.* Collaborates and integrates with other CMHA teams to ensure all services provided to client are communicated with clients whole treatment team* Assures timely documentation is maintained in accordance with CMHA policies and procedures, including but not limited to progress notes, treatment plans, authorizations, program orientation, and consent to treat, activity logs, and monthly targeted case management notes. Any individual needing assistance completing an online application should contact CMHA's Human Resources Department at*View email address on click.appcast.io.**Job ID#443*## Qualifications**Education**:Bachelor’s Degree in a mental health related field preferred or equivalent experience**Credentials and Licenses:*** Must have valid Connecticut driver’s license, a reliable vehicle Will become certified in MANDT, First Aid, and CPR.* Participate in trainings on an annual basis, as assigned.**Experience**:* Successful experience working in Case Management or related field with a solid understanding of Mental Health and co-occurring conditions, preferably a minimum of one year.* Beginner to intermediate experience with Microsoft Office products including Word, Outlook, Excel and PowerPoint* Experience in or the ability and willingness to learn how to use an Electronic Medical Record (EMR) system including: appointment scheduling, authorization tracking, assessments, treatment plans, progress notes and discharge summaries.**Competencies**: ***Behavioral Skill Sets**** Ability to assess and formulate case management needs as expressed by the client and document in treatment plans.* Effective interpersonal and communication skills with individuals with mental health and co-occurring disorders as well as with their families, caregivers and natural supports.* Capability to problem solve and use good judgement, work independently and as a team member.* Awareness/sensitivity to those we serve encourages and promotes a positive, productive and collegial environment, including culture, language, gender, sexual orientation, gender preference, physical handicap and spirituality.* Capacity to understand and incorporate recovery-based treatment, language, and interventions into documentation and daily operations.* Ability to teach, develop, achieve and motivate clients to maximum/desired behavior and/or goals.* Interpersonal ability to establish positive relationships with internal and external stakeholders.* Work a flexible schedule when needed.* Respond effectively to emergency situations.* Design, deliver, and ensure highly individualized services and support.
