What You'll Do

• Provide nursing care, education, and supervision for patients at home
• Collaborate with healthcare providers and community partners to coordinate care
• Evaluate patient and family responses to interventions and adjust care plans as needed
• Promote, maintain, and restore health, preventing illness and supporting rehabilitation
• Document care activities thoroughly and accurately
• Work independently under general guidance, following facility protocols
• Assist with care planning, problem-solving, and clinical decision-making

What You Need

• Valid RN license in Massachusetts (or eligibility)
• BLS and DEA certifications
• Minimum of [specify years if available] experience in community or home health nursing
