Physical Therapist
Seven Healthcare are seeking a dedicated Physical Therapist specializing in home health to join our reputable team in Seminole County, Florida. This full-time, field-based role offers the opportunity to provide high-quality care across multiple agency partners, ensuring a diverse and fulfilling work environment.

Key responsibilities of the Physical Therapist include:
- Conduct SOC (Start of Care), evaluations, reassessments, and discharges
- Coordinate visit times with patients and manage daily schedules
- Collaborate with PTAs for routine visits
- Complete documentation in partner agency EMRs, including Pariox, Kantime, Kinnser (WellSky), Axxess, and Homecare Homebase
Requirements of the Physical Therapist include:
- Valid Physical Therapy license
- Experience in home health settings preferred
- Proficiency with EMR systems
- Ability to cover Saturday orthopedic visits
