Hospice Nurse Practitioner

Role Hospice Nurse Practitioner providing palliative, symptom management, and supportive care to hospice patients. Responsibilities Conduct face-to-face patient visits, collaborate with physicians, develop and coordinate care plans, document clinical findings, and educate patients and families on self-care techniques. Maintain accurate clinical notes and communicate patient status to the medical team and administrators. Qualifications Active unrestricted license Current certification as a Nurse Specialist or Nurse Practitioner National certification (e.g., ANCC, NBCHPN, AANP) CPR certification At least 5 years of nursing experience Must have a valid driver’s license, reliable transportation, or access to public transit Preferred Skills Experience in oncology, hospice, geriatrics, or intensive care Palliative care certification Teamwork experience within interdisciplinary settings Compensation & Benefits Salary range from $109,500 to $164,000 plus bonuses, with comprehensive benefits, incentives, stock options, and 401(k). #J-18808-Ljbffr Vetted Solutions
