**City/State**Norfolk, VA**Work Shift**Third (Nights)**Overview:**# **Sentara Health is seeking an RN for it's Observation/Transitional Care Unit:*****The Obs/Transitional Care Unit is a 9-bed short stay unit used to care for med-surg patients who will be discharged within 24-48 hours.*****Position Status**: Full-Time Night Shift**Position Location:** Sentara Norfolk General**Minimum Requirements:*** RN Licensure - Virginia **Required*** BSN, RN-Associate's Degree OR RN-Diploma **Required*** Two years of nursing experience **Preferred*** Previous acute care experience **Preferred****Job Description Summary**Assesses, plans, develops, implements, and evaluates plans of care for specific patient populations. Coordinates and collaborates with a multidisciplinary team to facilitate appropriate and timely patient placement and/or transfer. Coordinates and communicates capacity management plan to nursing units, leadership team, and physicians.
