Job TitleCommunity Health Nurse LPNJob SummaryDuties of a Community Health Nurse LPN: provide direct patient care to include COVID related duties, conducting patient assessments, on-going case management, researching and locating additional resources, performing community outreach and education and acting as an advocate for appropriate patient care. The incumbent may perform duties in a variety of setting, in-home care, clinics, testing sites and immunization clinicsPrinciple Duties and ResponsibilitiesAssists with COVID testing, administering COVID vaccinations and documenting all encounters.Performs nursing tasks; provide patient care through home visits or clinical visits including: vital signs, nursing assessments (in collaboration with a RN), point of care testing, administering vaccinations and/or other injectable medications, medication management and education, development and implementation of a care plan and evaluating the need for supplies or ongoing patient education in addition to documenting in E.H.R.Assists with and coordinates educational classes related to community health.Identifies areas of concern and refers for further services to other departments.Act as an advocate for patient care during escorts to appointments and explaining diagnosis, treatment plans and follows up with the client.Monitor and track health care needs of assigned patients; works with other medical providers to coordinate proper and effective care.Coordinate referrals and follow up services with other Tribal departments; responds to patient and staff inquiries.Provides education for community members, promotes family health, helps prevent disease, and maintains health regimens.Document all encounters into E.H.R. system.Follows operating procedures and policies.Perform other duties of a similar nature or level as requested by supervisor or director.DURING THIS TIME OF THE COVID PANDEMIC, OUR MAIN FOCUS IS TESTING AND VACCINATION CLINICS. WE PROVIDE FULL PPE TO ENSURE OUR STAFF MEMBERS ARE SAFE DURING THIS TIME. OTHER DUTIES LISTED MAY OCCUR ON AN AS NEEDED BASIS.Required Knowledge, Skills, and AbilitiesKnowledge of:General nursing theories and practices;Tribal community health nursing care;Nursing services regulations and procedures;HIPAA and HPI regulations and standard procedures;Yaqui culture, customs, resources and traditions and/or a willingness to learn.Skills and Abilities:Clinical practices;Apply general nursing practices;Recognize and report patient health status;Respond appropriately to emergency situations;Follow written and verbal instruction;Operate a variety of office equipment, including a computer and related software applications;Good communication and interpersonal skills as applied to interaction with co-workers, supervisor, management, Council members, and the public. Have ability to sufficiently exchange or convey information and receive verbal and written work instructions.Education, Certifications and Experience RequiredLicensed as a Practical Nurse, plus two (2) of years nursing experience. ANDExperience as a nurse in a public health setting preferred;Must possess and maintain a valid Arizona Drivers License;Special Requirements:Current Arizona State License as a Practical Nurse (LPN); failure to maintain current license will result in termination;Current TB Test required;Current First Aid and CPR Certification;Must possess and maintain a valid Arizona Driver's License;Must have a current Level 1 Arizona Clearance Card; failure to maintain a current Level 1 Clearance Card will result in termination.BenefitsThe Pascua Yaqui Tribe provides 22 days of Paid Time Off which increases with years of service and 15 paid Holidays per year.
