Sapphire Center for Rehab & Nursing a 5 Star long term skilled nursing facility has immediate openings for enthusiastic and energetic  CNA’s  for all shifts.  We are looking for individuals who desire a worthwhile career that has purpose and makes a meaningful difference in someone’s life .   Under supervised professional and skilled guidance, our CNAs give direct nursing care to assigned residents and perform other services necessary for their personal and comforts needs. Job Responsibilities / Duties:

• Promotes resident hygiene and comfort
• Distributes food/water and feeds residents
• Assists with repositioning and transferring residents
• Ambulates and transports residents
• Records and totals fluid intake and output
• Weighs residents
• Documents activities on flow sheets
• Assists with admission, discharge and postmortem care
• Responds to call lights in a timely manner
• Performs perineal care and uncomplicated colostomy care
• Reports all observations & assessments to the Team Leader/Nurse Manager/Charge Nurse
• Takes and records vital signs
• Makes beds and keeps resident’s units clean and in order
• Prepares room for housekeeping after resident discharge

Skill / Experience Requirements .

Current NY CNA Certification.

Previous Long Term Care experience a plus.
