Responsibilities
- Deliver coordinated nursing care for a patient or assigned group in a home hospice setting, following established standards of care and the nursing process.
- Assess data, develop and implement care plans, and evaluate patient progress; adjust plans as needed.
- Collaborate with the patient, family, and interdisciplinary team to ensure a safe, healing, and compassionate environment; provide education to patients and families.
- Administer medications and treatments per orders; monitor responses and report adverse events.
- Supervise and direct activities of various nursing staff levels; delegate and coordinate duties as appropriate.
- Document care and outcomes in the appropriate electronic health record; participate in discharge or care-transition planning as applicable.

Requirements
- Current Registered Nurse (RN) license in the state of practice (California) or equivalent per state requirements.
- Basic Life Support (BLS) certification from an approved vendor; Allied facility-specific BLS certification requirements apply.
- Advanced Cardiovascular Life Support (ACLS) certification from an approved vendor; Hospital Fire and Life Safety (HLFS) training.
- EPIC EMR experience required.
- At least two years of recent nursing experience in the field.
- Education: BSN preferred.
- Ability to work night shifts and in a home healthcare/hospice environment; strong clinical judgment and independent decision-making.
