Duration: 3 Months Contract (Potential for extension)

Location: IL - Counties

Pay Rate:$40.00/ HR on W2

Shift Hours: M-F 8-5 CST

 

Job Summary

You will work as a  field-based case manager helping elderly and disabled members who are receiving  in-home care or nursing facility care . You will:

• Case load: 30–100 members
• Assess patient needs
• Create care plans
• Coordinate medical and social services
• Work with doctors, families, and providers
• Ensure members get proper, cost-effective care
• Focus: high-risk members needing ongoing support

Location & Travel

• Must live in  Illinois
• Target counties:
  • Cook
  • Lake
  • Will
  • Kane
  • Winnebago
  • Rock Island
• Field role (50–75% travel)to member homes. Miles will be paid 
• Remaining work is remote (home-based admin work)

Required License Must have ONE of the following:

• RN (Registered Nurse)with active unrestricted IL license
  OR
• LCSW (Licensed Clinical Social Worker)
  OR
• LCPC (Licensed Clinical Professional Counselor)

Experience Needed

• 3–5 years clinical experience preferred
• 2–3 years experience in:
  • Case management OR
  • Discharge planning OR
  • Home health coordination

Skills Required

• Ability to work independently (very important)
• Good communication + organization skills
• Comfortable working from home
• Computer skills (MS Word, Excel, Outlook, PowerPoint)
• Ability to use multiple systems

Education

RN, LCSW, or LCPC with current unrestricted IL state licensure REQUIRED
