Job Description

The Home Health RN is responsible for providing individualized, goal-oriented nursing care according to the nursing process and in accordance with a physician's order. This position also serves as Case Manager when leading the patient's care plan.

Responsibilities

• Provide individualized nursing care based on patients' needs and physician's orders.
• Use the nursing process to assess, plan, implement, and evaluate care for patients.
• Act as Case Manager, ensuring cohesive care coordination among multiple disciplines.
• Educate patients and families on health promotion and disease prevention.
• Document care provided and patient progress in compliance with regulations.

Required Experience / Certifications / Licensure

• Active Registered Nurse (RN) license.
• Experience in home health or community health care settings preferred.
• Strong organizational and communication skills.
