Collaboration About the Opportunity As a Primary Care Physician you will provide comprehensive outpatient primary care services to eligible Veterans through the Veterans Health Administration VHA Working within the Patient Aligned Care Team PACT model you will evaluate diagnose treat and manage a wide range of acute and chronic medical conditions while promoting preventive healthcare and wellness This role offers the opportunity to make a lasting impact on the health and well being of those who have served our country Schedule Mon Fri 800am 430pm What Youll Do Provide comprehensive outpatient primary care including preventive acute and chronic disease management Perform patient assessments medical histories physical examinations and develop individualized treatment plans Order interpret and communicate laboratory imaging and diagnostic test results Prescribe medications and monitor treatment effectiveness in accordance with VA guidelines Coordinate referrals and collaborate with specialists and multidisciplinary healthcare teams Promote preventive health through screenings immunizations wellness counseling and patient education Maintain accurate and timely documentation within the VA electronic health record Participate in quality improvement initiatives and maintain compliance with VA policies patient safety standards and privacy regulations Foster effective communication with Veterans their families and the healthcare team to ensure continuity of care Complete required VA credentialing privileging mandatory training and continuing medical education requirements Required Qualifications Active unrestricted Physician license Board Certified or Board Eligible in Internal or Family Medicine Current DEA registration National Provider Identifier NPI Current American Heart Association AHA Basic Life Support BLS and Advanced Cardiovascular Life Support ACLS certifications Successfully complete the AMI credentialing process Inclusion & Cultural Competency Utilize appropriate methods for interacting effectively and professionally with persons of all ages and from diverse cultural socioeconomic educational racial and ethnic sexual orientations lifestyles and physical abilities Deliver targeted culturally appropriate information to help individuals and groups understand health promotion and disease prevention information policies regulations
