Role Overview The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes, under supervision. Key Responsibilities Deliver high-quality clinical services within legal scope, following care plans and infection control protocols. Thoroughly document patient visits in electronic medical records and visit verification systems. Participate in care coordination by reporting patient needs, attending case conferences, and communicating with patients, families, physicians, and other healthcare team members. Qualifications & Skills Current, unrestricted LPN licensure in the practice state. CPR certification or ability to obtain within 90 days. Ability to work effectively in diverse home environments and manage multiple tasks independently. Valid driver's license, vehicle insurance, and access to transportation. Strong communication, organizational, and documentation skills are preferred.
