Our mission statement definitely speaks to what La Clinica de Familia stands for, which is to empower and enrich families, individuals, and communities by providing quality medical, dental, behavioral health and educational service for people of all cultures.Non-Exempt$16.37 to $22.00Job Summary:Under the supervision of the Director of Population Health, the Chronic Care Specialist is responsible for successfully supporting patients with high-risk health conditions to navigate the healthcare system. The Chronic Care Specialist interacts and collaborates with multidisciplinary care teams, to include physicians, nurses, pharmacists, laboratory technologists, social workers, and other educators to support the chronic care management/transition of care process. The Chronic Care Specialist acts as a resource for clinic staff.
