Atrium Health Transition Services Clinic in the greater Charlotte area is seeking experienced Nurse Practitioners and Physician Assistants to join our 100% outpatient Transition Services team. Work weekdays with 8am-5pm schedule and no nights or weekend shifts. You will join a team of 3 physicians and 2 APPs in a multidisciplinary ambulatory Internal Medicine clinic, providing post-discharge care, virtual visits, and care coordination to high-risk patients. #J-18808-Ljbffr Atrium Health
