RN - Registered Nurse - Hospital Case Manager

Location Geisinger Medical Center Annex Building Shift Days (United States of America) Scheduled Weekly Hours 40 Worker Type Regular Exemption Status Yes Job Summary Support patient care as a Case Manager at Geisinger Medical Center. Job Duties Team offers innovation and patient‑focused care. Work Monday‑Friday 8:00 AM‑4:30 PM with occasional weekend coverage (~every fourth weekend). At least two (2) years of prior RN experience is required. Ideal candidate will have prior RN experience in a hospital setting. Case Management experience is preferred and not required. Successful candidate must be available to work on‑site at Geisinger Medical Center (GMC). Voluntary benefits including accident, critical illness, hospital indemnity insurance, identity theft protection, universal life and pet and legal insurance Position Details Assess, plan, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the patient or member’s health status. Manage utilization and practice metrics to refine the delivery of care and maximize clinical, quality, and fiscal outcomes. Integrate evidence‑based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of patient‑centric treatment plans that promote quality and efficiency. Develop systems of care that monitor progress and promote early intervention in acute care situations. Assist with the design, implementation and evaluation of the advanced patient‑centered care model. Assess the healthcare, educational and psychosocial needs of patients or members. Design an individualized plan of care and foster a team approach by collaborating with the patient or member, family, primary care provider and other members of the health care team to ensure coordination of services. Continuously evaluate laboratory results, diagnostic tests, utilization patterns and other metrics to monitor quality and efficiency for the assigned population. Apply benefits and utilization management appropriately, serving as a resource to the patient or member and health care team. Maintain required documentation for all case management activities. Collect required data and use it to adjust the treatment plan when indicated. Education Graduate from Specialty Training Program‑Nursing (Required); Bachelor’s Degree in Nursing (Preferred). Experience Minimum of 2 years nursing experience (Required). Prior hospital RN experience preferred; case management experience preferred but not required. Certification(s) and License(s) Licensed Registered Nurse (Pennsylvania) – RN_State of Pennsylvania. Skills Computer literacy, critical thinking, organizing.
