Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist is responsible for evaluating all assigned patients and developing, implementing, and modifying treatment plans tailored to patient needs, goals, and physician direction. The therapist will oversee the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the creation of functional treatment programs to facilitate an effective return to community activities. This position also involves educating patients, families, students, coworkers, and self, while upholding the organization’s core values and standards.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and utilize clinical reasoning to formulate treatment plans focused on progression towards discharge.
• Ensure timely and accurate medical record documentation, including goal writing and functional quality indicator assessments.
• Participate in student programs, providing a positive learning environment through appropriate supervision and evaluation.
• Engage in ongoing professional development and continuing education to enhance clinical skills and contribute to organizational objectives.
• Maintain adherence to state practice acts and regulatory agency requirements, ensuring professional licensure is in good standing.
• Understand and contribute to organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited institution.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification required.
• Active and applicable physical therapy license for the state where the work will be performed.

Schedule

This is a full-time position with a 5x8 work schedule.
