Elders' Program Coordinator

Description The Elder Care Program Coordinator supports Tribal Elders by conducting home visits, assessing needs, and connecting them to medical, social, and community resources. This role serves as a liaison between Elders, their families, and service providers to ensure needs are met and services are coordinated. The Coordinator maintains documentation, supports care planning, and builds relationships within the Tribal community. Essential Duties Level I - Conduct home visits and welfare checks to evaluate and monitor Tribal Elders and their needs. Identify economic, social, cultural, physical, and environmental factors that impact functioning. Evaluate the effectiveness of the care plans and recommend adjustments as needed. Maintain outreach and provide support services to Tribal Elders based on individual needs. Conduct Activity of Daily Living Assessments to determine eligibility for program services. Connect Tribal Elders with essential community resources, providing hands‑on, in‑person assistance as needed. Prepare and submit payment requests and Elder In‑Home Care Timesheets for supervisor review and approval. Share information with Tribal Elders, families, and Tribal staff regarding relevant State and Federal resources, and build positive working relationships. Collaborate with medical professionals and other service providers to support coordinated care. Maintain transportation logs, reports, and related program documentation. Prepare purchase requests for program services, supplies, and activities for program manager review. Participate in Elder activities and events in a respectful and supportive manner. Maintain accurate client records and documentation. Participate in department meetings and trainings as appropriate. Level II - Performs all duties of Level I, plus: Assist with program policy development as needed. Collect and maintain accurate elder data and prepare reports on client demographics and services for monthly and annual reporting. Conduct follow‑up outreach for in‑home support and safety needs, including coordinating supplies and/or equipment rentals, and providing warm handoffs to other Tribal departments as needed. Develop and maintain professional relationships with State agencies and external organizations to connect Elders with appropriate support and resources. Additional Duties Assist in identifying Elder needs to ensure optimal support and care. Participate in program staff meetings, provider meetings, and workgroups as assigned. Attend trainings and continuing education as required to perform job duties. Assist with Elder Needs Assessment Survey, as requested. The Tribe values its commitment to community service and the well‑being of its members. Requirements Proficient in computer skills including Microsoft Office. Strong organizational skills with attention to detail and accuracy. Ability to prioritize workload and meet strict deadlines. Excellent customer service skills and written and verbal communication. Ability to communicate effectively with diverse populations in a courteous, clear, and helpful manner. Knowledge of Native cultures, history, and traditions preferred. Knowledge of age‑related health and social conditions affecting seniors preferred. Ability to maintain confidentiality and use tact and discretion. Demonstrate professionalism, strong communication skills, and sensitivity to the needs of Tribal Elders and their families. Education & Experience Requirements Education Required Level I - High School Diploma or GED Level II – Bachelor’s degree in social work, public health, human services, or related field; or an equivalent combination of education and experience that demonstrates the ability to perform the essential functions of the position may substitute for degree requirement. Experience Required Level I - Two (2) years of experience working with senior citizens is required. One (1) year of experience working in food preparation is preferred. Two (2) years of experience working in Native Community with Elders or Native families is preferred. Level II – Three (3) years of experience working with senior citizens is required. Two (2) years of experience working in food preparation is required. Two (2) years of experience supporting aging‑related health conditions or in geriatric social work. Three (3) years of experience working in a Native Community with Elders or Native families is required. Certifications / Licenses Must have and maintain a valid Washington State driver’s license. Current CPR/AED Certification. Must obtain a valid Food Handlers Certification within 90 days of employment. Other Requirements Must be able to pass a thorough pre‑employment background investigation. Must agree to be subject to the Tribe's Controlled Substance and Alcohol Testing Policy to include pre‑employment screening. Must be able to successfully pass the Tribe’s Defensive Drivers Class. Work Environment The work is performed in both an office setting and in the field, including frequent travel by car to conduct home visits and provide services to Tribal Elders. Tribal Preference The Snoqualmie Indian Tribe follows the Tribal Hiring Preference as outlined in tribal policies, including the Tribal Employment Rights Ordinance (TERO). TERO sets forth certain hiring preferences for qualified individuals who are enrolled in a federally recognized Indian tribe, as well as, in some cases, preference for certain family members of those individuals. Additional Information This is a full‑time hourly position and employment is at‑will. Compensation will be determined based on experience and starts at $27.21 per hour for level 1, and $31.56 per hour for level 2. Location Snoqualmie, WA Benefits First of the month following date of hire - competitive medical, dental, vision, life, and disability insurance benefits for all regular full‑time employees. Paid Holidays - Up to 18 paid holidays are offered every calendar year, which includes the workdays between Christmas Eve and New Year’s Day.
