About the Role VNAcare, a not‑for‑profit hospice care provider, is hiring an Admissions RN for its Encino location. The position is full‑time, day shift (10:00 a.m. – 6:30 p.m.) and offers a competitive salary, mileage reimbursement, a sign‑on bonus up to $5,000, and an extensive benefits package. Evaluate potential hospice patients and report findings to attending physicians; obtain orders for hospice care as appropriate. Cooperate with acute hospitals, skilled nursing facilities, and medical groups in the community to plan and coordinate hospice services for referred patients, arranging supplies, equipment, or other needs for discharge. Complete the hospice admission process, including required forms and documentation, and provide the agency and interdisciplinary team with information to ensure continuity of care.
