Accountable Care Manager

About This Role The Accountable Care Manager coordinates and monitors patient-centered quality care, ensuring optimal resource utilization and compliance with regulations. Highlights Pay: $118,749/year Location: Brooklyn, NY Facility: Woodhull Medical and Mental Health Center Department/Unit: PROFESSIONAL REV ORG Schedule: Full-Time, Day (9:00 A.M – 5:00 P.M) What You'll Do Review each patient's chart to ensure documentation supports the plan of care. Coordinate and participate in multidisciplinary rounds to discuss care plans. Collaborate with healthcare professionals to identify and eliminate barriers to care. Communicate with reimbursement departments and third-party payers for authorizations. Plan and implement strategies to reduce length of stay and improve patient outcomes. Initiate discharge planning by assessing patient and family needs. Coordinate timely implementation of discharge plans and follow-up care. Perform post-discharge phone calls to verify successful linkage to care. Maintain effective communication with all stakeholders involved in discharge planning. Engage with primary care physicians to support continuity of care. Requirements Education: Baccalaureate degree in Nursing or related health field from an accredited college or university required. Experience: Two (2) years of experience as a Registered Professional Nurse required. Licenses & Certifications: Valid New York State license and current registration to practice as a Registered Professional Nurse; Basic Life Support (BLS) certification required.
