A Place of My Own LLC is a NC DHHS licensed agency. We operate community programming and small 3/4-bed residential group homes for youth with developmental disabilities and mental illness. Our direct support professionals will coordinate and support client’s activities for daily living, volunteering, community integration, household management, community-based shopping and/or appointments; working closely with family persons and others to support the program person to progress toward more independent community living.

Need: 5 people

Hours: 20-32 hours per week

All shifts rotate to include every other weekend

midday shift between 11a - 11p

every other weekend day or night Sat/Sun 7a-7p or 8a-8p

Pay: $17/hour

Duties and Responsibilities:

1. Assist persons with all activities of daily living both in the home and in the community.
2. Assist persons with managing home, activities; following goals in the ISP.
3. Respond promptly to persons’ requests for assistance.
4. Assist with teaching appropriate social and independent living skills.
5. Encourage program persons to perform tasks as independently as possible, assisting them to participate in all activities to the maximum extent possible.
6. Accurately document provision of care as directed.
7. Stay current with changes in care/support (e.g. medications, treatments, behavior plans, etc.).
8. Alert program management in the event of a person’s change in status or behavior.
9. Assist with monitoring persons and gathering records and other data as requested.
10. Complete all documentation and Incident Reports within program timelines.
11. Actively participates in person, family, team, and staff meetings.
12. Attend trainings as assigned and maintain certification of required trainings (including but not limited to CPR, First Aid, and Medication). Must successfully complete medication administration competency exam.
13. Transport in personal vehicle or company vehicles.

Qualifications:

• A high school diploma or equivalent, and successfully complete the DSP curriculum.

• Ability to understand, speak, read, and write fluent English; sensitive to persons’ needs.

• Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations, can communicate effectively, use good judgment, high energy level, function well in a team atmosphere; perform basic computer functions.

• Must be able to perform each essential duty satisfactorily.

• Must have a valid driver’s license and access to an operational and insured vehicle in addition to willingness to use it to transport persons.
