Shifts may consist of 8 or 9 hours per day. Qualifications\n \n Education: Master's Degree from an accredited physical therapy program \n Experience: Experience working directly with patients in a long-term care or hospital setting \n Licenses/Certifications: Current Physical Therapist state license, Current Basic Life Support (BLS) certification or ability to obtain within 6 months of hire \n Knowledge/Skills/Abilities: Strong documentation skills, excellent verbal communication, satisfactory patient service, teamwork, and ability to provide age-appropriate care. \n Position Summary\n The Physical Therapist is responsible for planning and executing rehabilitation programs aimed at helping patients regain physical function following an accident or surgery. The therapist also aids in managing or alleviating pain associated with degenerative conditions or terminal illnesses. Utilizing a combination of physical activities, reflexology, massage, and manual therapy techniques, the Physical Therapist collaborates with other healthcare professionals to ensure optimal patient outcomes. The patient load is comprised of approximately 90% outpatient and 10% inpatient cases. Responsibilities\n \n Consult with patients to assess their physical condition and symptoms, reviewing medical histories and referrals from providers. \n Diagnose movement dysfunction through patient observation and consultation. \n Develop individualized treatment plans tailored to each patient's needs. \n Instruct patients in therapeutic exercises and stretches to enhance their recovery. \n Administer hands-on therapies, such as massage, to alleviate pain and stimulate healing. \n Maintain detailed records of patient conditions, care plans, therapies, and progress. \n Communicate progress updates and current status to relevant parties, including family members and healthcare providers. \n Perform additional duties and responsibilities as assigned. \n Professional Communication\n \n Uphold confidentiality pertaining to patient, family, and staff information. \n Engage with patients and families from diverse developmental and sociocultural backgrounds. \n Provide informative support to patients and families to ease anxiety, demonstrating an attitude of acceptance, sensitivity, and caring. \n Maintain professional relationships while conveying pertinent information to other healthcare team members and referral agencies. \n Effectively communicate clinical priorities and concerns with peers and management. \n Teamwork\n \n Engage in assignments based on priorities, competencies, and unit needs. \n Work collaboratively with other care providers to complete tasks efficiently. \n Communicate patient condition updates to necessary team members, including physicians and nursing staff. \n Utilize problem-solving and conflict resolution skills to cultivate effective working relationships. \n Report to work on time and as scheduled. \n Professional Development\n \n Participate in in-service training, continuing education, and departmental meetings. \n Contribute to annual peer performance reviews as requested by supervisory staff. \n Identify and address opportunities to enhance patient care and customer service. \n \n \n Wellspring Nurse Source Job ID 37821427.
