Job Description:

Are you a dedicated Physical Therapist looking to make a difference in long-term care settings? Join our team to provide exceptional patient care in a rewarding environment. In this role, you will evaluate, rehabilitate, and develop individualized care plans for patients with physical functioning disorders and wound care needs. You will utilize evidence-based assessments and procedures including manual techniques, ambulation, therapeutic exercises, modalities, supportive devices, and wound management. Your responsibilities include tracking patient progress, adjusting treatments, and evaluating outcomes to optimize recovery.


You will provide consultation and counseling to patients, families, caregivers, and other healthcare providers. Effective communication with your team is essential to coordinate care and ensure seamless services. Additionally, you will document all clinical interventions, treatment minutes, and patient progress in accordance with regulatory standards. Participation in meetings, trainings, and conferences will support your ongoing professional development. You will supervise and support rehabilitation team members including assistants, aides, and students, ensuring adherence to APTA and state practice acts, and maintaining ethical and professional standards.
