Pediatric Nurse

Job Overview Under limited supervision, the licensed practical/vocational nurse (LPN/LVN) is responsible for the delivery of patient care to assigned patients utilizing the nursing processes of assessment, planning, implementation, and evaluation in accordance with federal, state, and local laws, and any standards, policies, procedures, guidelines and regulations that govern any facility the LPN/LVN is working at. Duties and Responsibilities • Provides skilled nursing as ordered on the plan of treatment and nursing care plan.• Each shift of visit reflects the provision of skilled nursing care rendered in accordance with physician’s orders.• Identifies new problems or changes, follows through with Director/Supervisor notification, and obtains necessary orders for intervention(s) per state regulations.• Documents all relevant information on clinical notes, supplemental physical orders and nursing care plan.• Observes aseptic technique as required and performs specific treatments and medication administration as per the care plan.• Functions as a team member under the direction of the registered nurse.• Communicates skilled observations and significant changes in patient status to the Director of Clinical Services and/or Clinical Manager.• Requests modifications or additional orders based on the patient’s clinical status and skilled nursing judgment.• Documents all physician collaboration in the medical record and demonstrates appropriate utilization of all available services.• Participates in the agency’s other mandated activities such as InServices.• Adheres to all agency policies, procedures and the handbook, completing all clinical documentation accurately and on time.• Represents themselves in a positive and professional manner in the company and community, adhering to dress code, and reporting on time to complete work within designated time. Requirements Graduation from an approved certification/program/accredited school/undergraduate/graduate school or post graduate education to reflect ability to perform job as described. Holds a valid/unencumbered verification/license to practice the job as described in the state where services are provided. Professional experience as required per state regulations to perform the job as described. Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater. Reliable transportation. Provide copy of current TB results or Chest x-ray. Provide copy of current CPR certification as per agency policy. Be flexible, organized, and able to exercise independent and sound judgment to perform the job duties and responsibilities. Physical Requirements Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms and other physical activities as needed by job duties. Vision and hearing within normal limits, with or without aid. Ability to frequently lift and move up to 15 pounds and occasionally lift or move 100 pounds or more if applicable. Ability to perform the anticipated level of physical assistance required to provide safe client care; if greater than the employee can safely provide, the supervisor should be notified immediately. Work Environment The work environment is highly variable, as care is generally provided in private homes. Lighting, acoustics, air quality, temperature, and environmental hazards are not predictable. The employee is occasionally exposed to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
