This 36-hour/week role is primarily daytime, mobile/field-based, and built around patient visits-not a traditional office-based shift. Description :


Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

Education/License:


Diploma from an accredited school/college of nursing. Required professional licensure at time of hire.

Responsibilities :

• Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.|
• Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.|
• Carries out physician and nursing orders. Attends team meetings and care plan meetings.|
• Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.|
• Participates in after hours/weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.

The payrate for this position is from $40.00 - $54.12 per hour.
