McLaren Health Care in Michigan seeks an experienced Registered Nurse Care Manager to coordinate care transitions and optimize outcomes across the continuum. You will work with physicians, nursing staff, social workers and other resources to prevent readmissions and support appropriate level-of-care decisions. Required: RN licensure, Bachelor’s in nursing, and at least three years of acute hospital care.
