We are seeking a Home Health Aide to become a part of our team! You will provide routine healthcare to patients at the patient's home or in a care facility.

Responsibilities:

• Provide routine individualized care to the elderly, convalescents, or persons with disabilities
• Monitor and report changes in patient health status 
• Provide personal care, such as bathing and dressing patients
• Provide specialized healthcare such as administering medication and changing bandages
• Teach family members ways to care for patient

Qualifications:

• Experience in hospice, home health, or other related fields
• Positive and patient demeanor
• Ability to build rapport with patients
• Ability to handle physical workload
• Excellent written and verbal communication skills

Services may be requested up to 24 hours per day, 7 days per week in the home. Our Care Coordinators will develop a personalized Home Plan of Care that is patient centered to include individualized needs and acuity level. It is our mission to maintain the safety and well-being of each and every person we serve by maintaining standards we created that exceed far above the required expectations set forth by our State and Federal Guidelines. We provide these services through programs including the VA Homemaker/Home Health Aide Program, EEOICPA, FECA and Black Lung.
