Care Manager (CNA/CHHA) Part Time (3p-11p) w/ every other weekend

Job Overview Assists residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities. Responsibilities and Duties Provide resident care including bathing, dressing, toileting, and feeding as outlined in the care plan. Maintain the safety, comfort, dignity, and confidentiality of all residents. Escort residents to meals and activities. Ensure all assigned residents are clean, neatly dressed, groomed, safe, and fed during assigned shifts. Document all services provided to assigned residents during the shift. Report any change of condition in a resident immediately to the supervisor. Qualifications Certified Nursing Assistant or Certified Home Health Aid. Knowledge and experience working with the elderly population; preferred dementia experience. Physical Abilities Requires standing, walking, sitting, using fine and gross motor skills, reaching with hands and arms, balancing, stooping, kneeling, crouching, talking, hearing, and smelling. Must lift or carry up to 50 lbs; intermittent physical activity includes lifting and supporting residents. Proper body mechanics must be used.
