Description Location: UCHealth Longs Peak Hospital, Longmont, CO Department: LPH Pharmacy Work Schedule: Full Time, 80.00 hours per pay period (2 weeks) Shift: Day and Evening Rotation Pay: $59.80 - $83.72 / hour. Pay is dependent on applicant's relevant experience This position is an onsite role and does not offer a hybrid or remote option Minimum Requirements Bachelor's Degree in Pharmacy from an accredited school of Pharmacy. State licensure as a Pharmacist and any relevant life support certification and/or relevant licensure/certification as determined at position level. Responsibilities Provides top‑of‑scope pharmacy support utilizing evidence‑based protocols and best practices to promote safety at the point of care. Values and effectively partners with multidisciplinary team to achieve care outcomes. Prioritizes wellness, patient perspective, and evidence‑based practice. Models proficiency through precepting and mentoring those new to healthcare and/or UCHealth. Welcomes new knowledge in a fast‑paced, innovative clinical environment. Contributes proactively to solutions to secure safety and quality at the point of care. Ensures medications are prescribed, accurately dispensed, and administered according to best practices. Provides drug information and counseling to patients and caregivers regarding medication use, accurate dosage, potential side effects, and administration instructions. Stays current with updated medication formularies, new drug therapies, treatment protocols, and emerging medical research through continuous professional development and clinical education. Communicates with physicians, nurses, and other healthcare professionals to facilitate collaborative care and ensure optimal outcomes.
