Community Case Manager

Clinical Professional Full Time , Days Job Summary: Case Managers are responsible for coordinating and assuring the continuity of services for each patient on their caseload. Working with each client and caregiver(s) (if appropriate) to coordinate services, ensuring linkage and advocacy. Meet with clients/caregiver(s) at intake, periodically and annually to obtain information to conduct a comprehensive assessment of needs. During periodic meetings, revise case management plans to document any newly identified needs and /or services. Promote and ensure a person-centered planning approach to care and individual choice of services. Complete all documentation and billing in compliance with standards set by state and agency policy. Attend meetings, training, and in-services as required and participate in regularly scheduled supervisions as required. Review records, summaries and assessments from current providers. Refer and monitor clients services as needed. Communicate, coordinate and document client and caregiver (as appropriate), and providers as needed and document appropriately. Attend clinical meetings to discuss patients as needed. Meet with internal providers, no less than quarterly to obtain information to complete Quarterly Service Review documentation. Attendance is an essential function of this position. Must be able to establish a rapport with the patients. Must treat all patients with dignity and respect. Provides input into program issues and shares/receives feedback in a healthy, constructive atmosphere. Has general knowledge about medications and diagnosis as well as a working knowledge of issues,rehabilitation and supportive living concepts associated with adult clients and uses these appropriately with clients. Has working knowledge of crisis intervention techniques and counseling principles and remains current. Is able to organize and prioritize a workload so that rehabilitative and administrative duties are completed on time and in accordance with productivity and quality standards. Learns the philosophy and operating procedures of Lakes Region Mental Health Center and applies them effectively both within and outside the agency. Represents the clients/program/agency constructively both within and outside the agency. Maintains general knowledge of community resources. Ability to use computers and word processing equipment. Ability to express or exchange ideas by means of the spoken word and to receive detailed information through oral communication. Primary functions include activities in which verbal communication with clients and with others is key to job effectiveness. Visual acuity must meet a minimum standard to work with preparing,reading and analyzing data and reports, work with computer terminals, etc. Education and Experience: Bachelor's in mental health counseling, social work, psychology, nursing, counseling (preferred) or other related field with two (2) years of experience preferably in mental health. Meets qualifications to provide Functional Support Services as specified in He-M426-11 and He-M426-13. Must have and maintain a valid NH driver's license with current liability insurance which meets Lakes Region Mental Health Center requirements. Physical Requirements: Is able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 lbs. or more. Is able to tolerate periods of heavy workload or excessive stress.
