This role includes performing vital signs and observations, assisting with admissions and discharges, supporting daily living activities, maintaining unit cleanliness, and updating documentation within the Withdrawal Management and Intensive departments/ph3Essential Duties and Responsibilities/h3ulliPerforms and documents all vital signs accurately and promptly./liliPerforms and documents hourly observations promptly./liliPerforms and documents fall risk observations promptly./liliResponsible for the admission process for both the Withdrawal Management and Intensive units, including the search, storage and documentation process./liliResponsible for the discharge process for both the Withdrawal Management and Intensive units, including proper handling of patient belongings and documentation process./liliEnsures patients receive all possessions and medications upon discharge./liliResponsible for updating the communication log, census, and other misc. shared documents/liliAssists patients with ADLs as needed./liliResponsible for registration of new patients to both programs/liliResponsible for the nightly process in Epic, if applicable/liliResponsible for managing the doctors list of physicals and preparing the patients to see the doctor/liliMaintains cleanliness of the unit, kitchenette, patients rooms, and triage./liliResponsible for maintaining a therapeutic, peaceful, clinical treatment environment/liliThorough shift-to-shift report received and given before and after each shift./liliAll other duties as assigned./li/ulp*All agency staff must be awake during all scheduled hours*/pp*All agency staff are required to attend all mandatory department/agency meetings and trainings*/ppSchedule:/pp3:00 PM - 11:30 PM/ppWeek 1: Monday, Tuesday, Wednesday, Thursday, Saturday/ppWeek 2: Sunday, Tuesday, Wednesday, Thursday, Friday/ppRequirements:/pulliAn active CNA/MA/PCA Certification from the State of Connecticut (preferred), or equivalent experience./liliCPR/First Aid./liliStrong communication and observation skills./liliCompassion, patience and a dedication to patient care./liliComputer literacy and typing skills./liliAbility to work effectively with a team./li/ul/div
