Behavioral Health Therapist
Pregnant With Possibilities
Position Summary The Behavioral Health Therapist is a licensed behavioral health professional responsible for providing high-quality therapeutic services to individuals receiving care through Bloom at PPRC. The Therapist maintains an assigned clinical caseload and provides individual, family, and/or group counseling using evidence-based, trauma-informed, culturally responsive, and client-centered practices. As part of the Therapist’s clinical responsibilities, this position also supports Bloom’s clinical intake and assessment process. The Therapist conducts comprehensive behavioral health assessments, evaluates clinical risk, identifies presenting concerns and treatment needs, develops individualized treatment plans, and determines appropriate recommendations for ongoing care. When clinically appropriate, the Therapist may continue providing therapy to clients initially assessed through the intake process, promoting continuity of care from assessment through treatment. The Therapist collaborates with the Clinical Director, behavioral health providers, PPRC staff, healthcare professionals, and community partners to coordinate care, facilitate referrals, support Bloom programs and initiatives, maintain timely and compliant clinical documentation, and promote positive client outcomes. The position requires sound clinical judgment, strong therapeutic and interpersonal skills, professional boundaries, accountability, and adherence to ethical, licensing, regulatory, accreditation, organizational, and billing requirements. Core Areas of Responsibility Therapeutic Services & Caseload Management Maintain an assigned clinical caseload and provide individual, family, and/or group counseling within the scope of professional licensure. Establish positive, trusting therapeutic relationships while maintaining appropriate professional boundaries. Utilize evidence-based, trauma-informed, culturally responsive, and client-centered therapeutic approaches. Deliver therapeutic interventions aligned with individualized treatment goals and identified client needs. Continually assess client functioning, progress, barriers, and emerging needs and modify interventions as clinically appropriate. Educate and involve family members, support persons, or other appropriate individuals in treatment when clinically indicated and authorized by the client. Maintain established productivity expectations for assigned caseload and service delivery. Provide services in approved settings, which may include the Bloom/PPRC office, community-based locations, partner sites, and telehealth as appropriate. Clinical Intake, Assessment & Treatment Planning Complete comprehensive behavioral health assessments within established organizational timeframes. Assess presenting concerns, client strengths, behavioral health history, psychosocial needs, risk factors, functional needs, and treatment priorities. Administer and document required clinical and program screening tools, including the Edinburgh Postnatal Depression Scale (EPDS) and other approved measures. Complete diagnostic assessments and document diagnoses when within the scope of licensure and organizational policy. Conduct suicide and other clinical risk assessments when indicated and determine the appropriate level of response, consultation, referral, or follow-up. Develop individualized, measurable, and client-centered treatment plans based on assessment findings and identified clinical needs. Review and update treatment plans in accordance with clinical, regulatory, payer, and organizational requirements. Establish appropriate recommendations for therapy, referrals, care coordination, and additional services based on assessment findings and consultation with the Clinical Director as appropriate. Maintain a minimum 95% on-time completion rate for assigned assessments. Clinical Documentation & Regulatory Compliance Complete assessments, treatment plans, progress notes, discharge documentation, and other required clinical records within established timeframes. Maintain complete, accurate, confidential, and audit-ready client records that support clinical decision-making and services rendered. Ensure documentation meets applicable ethical, licensing, regulatory, accreditation, organizational, payer, and billing requirements. Maintain required client consents, releases of information, and supporting documentation. Achieve a minimum 95% compliance rating during applicable chart reviews and documentation audits. Maintain strict confidentiality and comply with applicable privacy requirements and organizational policies. Risk Management, Safety & Ethical Practice Identify and respond appropriately to clinical risk, safety concerns, crisis situations, and changes in client presentation. Develop, implement, and document client safety plans when clinically appropriate. Fulfill mandated reporting responsibilities in accordance with applicable law, professional standards, and organizational procedures. Consult with the Clinical Director regarding high-risk situations, safety concerns, ethical dilemmas, complex clinical presentations, and scope-of-practice questions. Ensure identified risks receive appropriate intervention, documentation, consultation, referral, and follow-up. Practice within the scope of professional licensure and maintain compliance with applicable ethical and professional standards. Care Coordination & Continuity of Care Coordinate client care with healthcare providers, social service agencies, community organizations, and other appropriate partners. Facilitate referrals and resource connections that support treatment goals and identified behavioral health or psychosocial needs. Maintain appropriate releases of information and documentation of collaborative care activities. Participate in interdisciplinary case consultation and treatment-planning discussions. Promote continuity of care through timely communication, follow-up, referrals, and coordination with internal and external providers. When a client requires services beyond Bloom’s scope or the Therapist’s scope of practice, facilitate an appropriate referral and support a clinically responsible transition of care. Bloom Program Delivery & Participant Engagement Facilitate and document assigned Bloom behavioral health programs and initiatives, including H.O.P.E., THRIVE, therapeutic groups, support groups, workshops, and other designated services. Prepare for assigned sessions and maintain fidelity to approved curricula, clinical protocols, and program requirements. Complete attendance tracking, assessments, evaluations, participant surveys, and outcome measures within established timeframes. Conduct timely outreach following missed therapy appointments, groups, or program sessions in accordance with organizational procedures. Implement engagement and retention strategies that support consistent participation in treatment and programming. Support program outcomes, grant requirements, quality-improvement initiatives, and organizational reporting expectations. Client Experience & Professional Practice Foster a welcoming, supportive, culturally responsive, and trauma-informed therapeutic environment. Respond professionally and appropriately to client concerns, needs, and feedback. Communicate effectively with clients, colleagues, leadership, healthcare providers, and community partners. Maintain all professional licenses, credentials, continuing education requirements, and professional liability coverage applicable to the position. Demonstrate accountability, initiative, adaptability, collaboration, sound time management, and commitment to continuous quality improvement. Knowledge of trauma-informed care, evidence-based therapeutic practices, clinical documentation, mandated reporting, professional ethics, and confidentiality requirements. Demonstrated ability to maintain accurate and timely clinical documentation and manage competing clinical priorities. Strong interpersonal, communication, assessment, organization, clinical judgment, and relationship-building skills. Ability to work independently while collaborating effectively within an interdisciplinary behavioral health environment. Preferred Qualifications Independent Ohio licensure (LISW or LPCC) demonstrated progression toward independent licensure. Experience providing behavioral health services to pregnant and postpartum individuals, parents, families, or other populations experiencing barriers to care. Experience within nonprofit, community mental health, maternal/perinatal health, or integrated-care settings. Familiarity with behavioral health quality assurance, billing documentation, chart audits, and accreditation requirements.
