Licensed Practical Nurse (LPN)

Do you want a nursing role where your clinical skills directly improve the lives of others? As a Licensed Practical Nurse (LPN) , you’ll provide hands-on nursing care and support to individuals with developmental disabilities, acquired brain injuries, substance use challenges, and emotional or behavioral needs.

This is a dynamic role where no two days are exactly alike . You’ll travel between multiple locations, collaborate closely with nurses, physicians, caregivers, and other professionals, and serve as a trusted clinical resource for individuals and the teams who support them.

What You’ll Do

• Provide prescribed nursing care, medical treatment, and personal care services to individuals served.
• Complete health assessments and communicate findings to the RN or physician to support individualized nursing care plans.
• Follow up on physician orders and coordinate care related to medical and dental appointments.
• Administer, monitor, and document medications accurately and promptly while serving as a liaison with pharmacies.
• Review medication errors, identify trends, and recommend improvements to medication procedures and practices.
• Conduct quality assurance reviews of healthcare records to ensure accuracy, compliance, and continuity of care.
• Protect confidentiality, uphold individual rights, and follow universal precautions, infection-control practices, company policies, and applicable regulations.
• Collaborate with external healthcare providers and agencies to coordinate services and promote consistent, quality care.
• Administer and maintain required health testing and documentation for staff and individuals served.
• Train staff on important healthcare topics, including medication administration, universal precautions, and infection control.
• Educate individuals served on topics such as self-medication, prescribed treatments, STD prevention, and smoking cessation.
• When needed, assist with household activities such as light housekeeping and meal preparation and provide transportation to activities and appointments.
• Participate in the on-call rotation and provide after-hours emergency coverage as needed.

What You’ll Bring

• Associate degree in Nursing or a related field OR graduation from an accredited LPN program.
• At least one year of nursing experience working with the population served.
• Current state LPN license and any additional certifications required by state regulations, such as CPR/First Aid, HIV/AIDS, Bloodborne Pathogens (BBP), and ACT training.
• Current driver’s license, vehicle registration, and auto insurance if providing transportation to individuals served.
• Strong communication skills and the ability to build positive relationships with individuals, families, coworkers, and healthcare professionals.
• Self-motivated, organized, and detail-oriented with the ability to manage multiple priorities.
• Reliable, responsible, compassionate, and committed to making a meaningful difference.
