Hospice Community Liaison

MissionHires is seeking a Hospice Community Liaison on behalf of a healthcare organization to build and maintain legally compliant referral relationships that grow hospice census. The role serves as the point-of-contact between referral sources, patients/families, and the hospice team to coordinate admissions, educate on hospice benefits and eligibility, and support broader marketing and branding goals. This is a field-based position requiring regular travel to hospitals, skilled nursing facilities, assisted living communities, and physician offices. How You’ll Contribute Develop and expand legally compliant relationships with new and existing referral sources to drive hospice admissions Maintain and broaden the number of referral sources within the service area Collaborate with referral source staff to collect clinical, financial, social, and psychological information for admissions review Work directly with families and the hospice team to complete admission paperwork and facilitate the admission process Coordinate with insurance liaisons and internal admissions personnel for efficient admissions processing Serve as a knowledgeable resource on the hospice benefit, including eligibility requirements and program specifics Ensure all written materials provided to referral sources and potential patients are clear, correct, non-deceptive, and fully informative Complete timely and accurate documentation of sales/marketing activities in designated software systems Partner with leadership on broader marketing and branding initiatives, including an annual calendar of campus-wide events Adhere to confidentiality and HIPAA requirements and demonstrate cultural competency with diverse families and staff Comply with applicable laws, regulations, and guidance (e.g., OIG hospice compliance program guidance) governing hospice marketing and admissions Uphold ethical standards by avoiding prohibited incentives, improper services, and inappropriate record review Follow all safety, sanitation, infection control, and universal precaution policies and procedures Report hazardous conditions, accidents, and incidents to the immediate supervisor Perform other duties as assigned What Makes You a Great Fit Two years of sales and marketing experience in hospice, long-term care, senior living, or hospital settings Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license preferred Excellent written and verbal communication skills; presentation experience preferred Proficiency in Microsoft Word, Excel, and PowerPoint Valid driver’s license and ability to drive for long periods Ability to lift and push/pull up to 25 lbs routinely; ability to stand for entire shift with bending, pulling, and pushing Understanding of confidentiality, HIPAA, and cultural competency principles Commitment to ethical marketing and compliance with laws and guidance related to hospice admissions and sales activities Willingness to travel weekly to hospitals, nursing facilities, assisted living communities, and physician offices Ability to work in environments with potential exposure to infections, diseases, equipment-related risks, and odors Your impact is rewarded.
