Overview Value-based Advance Practice Provider role managing chronic conditions of high-risk geriatric patients Monday-Friday, no weekends or evenings, in a supportive team setting. This position is w-2 salaried, includes benefits, plus annual quality-based performance. The Nurse Practitioner who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient’s needs.
